The old method for changing the style of the text on a Web page was to edit each and every font tag on the page. With CSS changing the color of the text used on your MySpace profile page is simply a matter of changing the value that defines the font color in the CSS rule for the body tag on your page.

5. Tweaking & experimenting — the best way to get started creating your own MySpace layout is to grab a free layout you like and start experimenting. Don’t be afraid – just jump in with both feet and see what happens when you start changing around the values in the CSS section of your layout. Subdomains and WordPress – ‘How To’

This article resulted from some recent work performed on a new website, and I have written it as a step-by-step solution. It could save you a stack of time. It assumes that you understand the use of FTP.

AIM 1: to create a website and to add a subdomain.

Please note: the domain, subdirectories and files in this article are imaginary, and are for instructional purposes only.

Let’s use the example of a fruit grower.

Build a website fruit.com using your FTP client software, and with the files and subdirectories as follows –

  • fruit.com/index.html
  • fruit.com/apples/orchard.html
  • fruit.com/grapes/vineyard.html

Where fruit.com is usually represented by /public_html/ on the server, and index.html is the home page.

SUBDOMAIN

Use the cpanel utility to ‘add’ a subdomain – citrus.

This means you will be creating a new folder ‘citrus’ which is a SPECIAL FOLDER.

Using FTP again with the username and password as before (i.e. you don’t create a new FTP user for the subdomain), build –

  • /public_html/citrus/index.html (and this time it’s a different index.html)
  • /public_html/citrus/oranges/grove.html
  • /public_html/citrus/grapefruit/orchard.html

To access index.html, grove.html, and orchard.html using your browser, the urls are –

  • citrus.fruit.com/index.html
  • citrus.fruit.com/oranges/grove.html
  • citrus.fruit.com/grapefruit/orchard.html

If everything has been set up correctly, the following WILL NOT display the new index.html, or grove.html, nor orchard.html

  • fruit.com/citrus/index.html
  • fruit.com/citrus/oranges/grove.html
  • fruit.com/citrus/grapefruit/orchard.html

Conclusion: designating the folder ‘citrus’ as a subdomain has made it special. It is special in that you have to access the folder in a special way. It is not special in the way you use FTP to add content.

WORDPRESS (abbrev. ‘WP’)

AIM 2: to install WP on the site fruit.com, and then on the subdomain ‘citrus’.

On your main site –

Using cpanel’s utility for managing MySql databases, create a database, say, ‘wp’. It’s actual full name you will see will turn out to be ‘yourusername_wp’.

Create a new user, say, ‘wpu’. His actual full name will turn out to be ‘yourusername_wpu’.

Connect the new user to the new database.

The actual names will be the ones to use in your WP installation when you edit your wp_config file (the ‘wp_’ here has nothing to do with the name you gave to your database).

Using FTP, install WP, following the installation method, on your main site fruit.com. You will probably install it in /public_html/, or in something like /public_html/blog/

Now for WP in your subdomain –

Using cpanel’s utility for managing MySql databases, create a database, say, ‘ws’. It’s actual name will turn out to be ‘yourusername_ws’.

Create a new user, say, ‘wsu’. His actual name will turn out to be ‘yourusername_wsu’.

Connect the new user to the new database.

The actual names will be the ones to use in your WP installation when you edit your wp_config file.

Using FTP install WP, following the installation method, in your folder ‘citrus’. You will probably install it in /public_html/citrus/, or in something like /public_html/citrus/blog/

Well done! You now have a working subdomain with WordPress installed on the main domain and in the subdomain.

Quito is the capital city of Ecuador and also the second most populous city after Guayaquil. Better Choices Than a Nursing Home or Hospital-A True Story!

It’s been a month since my wife, a former nurse yielded to three phone calls from a friend to get her out of the nursing home. When she came to us, she needed a wheel chair–she couldn’t walk, talk or smile. Now she has all three and her humor rewards us!

The average number of prescriptions in a nursing home is 9-13 and these figures are nearly a decade out of date. The benefit to the patient is very low–they are prescribed for the nurses who don’t have to put up with any complaints; the patient is zonked out.

A patient can stand in the hall and fill his or her diaper while staring into space. They will say everything is fine. No brain, no pain, as we said in medical school.

Adverse drug reactions have made medical care a leading cause of death. These reactions put 2.2 million people in hospitals and 106,000 died, “making these reactions between the fourth and sixth leading cause of death.” Journal of American Medical Assoc. 4-15-1998

But how many died at home? 199,000 according to the Western Journal of Medicine… June, 2000. Deaths in and outside hospitals from Rx totaled 305,000 then, with 8 million admissions to the hospital and 3 million for long-term care (nursing homes–these people were messed up for life!)

“From 1998 through 2005, reported serious adverse drug events increased 2.6-fold… fatal adverse drug events increased 2.7-fold… ” Archives of Internal Medicine, Sep 10, 2007, p 1752.

This information makes prescription drugs the #1 cause of death, but we still hear “heart disease… heart disease… heart disease.” That’s a smoke screen for the real culprit, and it justifies giving more drugs for cholesterol, blood pressure, diabetes control when ALL of those conditions are diet related, but never mind–eat as you please and take the prescriptions?

When a patient dies in a nursing home (and half of them do within a year of admission) the doctor signs the death certificate as dying from heart disease in most cases. The heart stopped, but no autopsy is done to discover the coronary arteries are OK and it was probably the drugs that affected the heart rhythm.

So how did Miriam do so well at our place? It’s mainly a low fat vegan diet and none of the prescriptions that she was taking in the nursing home! And her blood sugars (finger stick device) have been lower than at the nursing home. A side benefit to Miriam–only once a day compared to four finger sticks daily in the nursing home.

Nurse Jackie’s Hospital Administrator

Nurse Jackie is one of Showtime’s hottest shows this season. Featuring Edie Falco, an HBO veteran from the hit The Sopranos, as an amoral emergency room nurse dedicated to saving lives, while cheating on her husband and indulging in powerful painkillers to combat her back pain, the show transcends typical medicine dramas popular in primetime television by focusing on those who heal, not those who diagnose.

This Showtime drama also features The West Wing’s Anna Deavere Smith as Gloria Akalitus, a nurse-turned-hospital administrator. Akalitus is the butt of many a joke in the show, getting giddy off of Jackie’s Percocet-laced coffee, accidentally tazering herself in an elevator, and questioning a gay nurse about how to obtain adoption papers for a child left at a nursing station.

Where lies the line between fact and fiction in the characterization of this hospital administrator? Frankly, the line zig-zags. Hospital administrators play an important part in the inner workings of the hospital, and help to keep everything running smoothly. But in a dark comedy like Nurse Jackie, Akalitus is a walking punch line, providing a much needed dose of laughter amidst so much darkness. However, if her employees behaved themselves, Akalitus would most likely seem like an average nursing supervisor.

In one notable episode, Akalitus urges Jackie Peyton to make a dying man vacate a bed due to overcrowding. Jackie asks her to look at the man again, not as a healthcare administrator, but as a nurse. Akalitus allows Jackie until the end of the day to find him another place to stay, because she realizes that he’ll be dying soon.

This aspect of Akalitus’ character may be legitimate. It would be logical for a nurse to become a supervisor. Many hospital administrators began as healthcare workers. However, it’s not a simple jump from nurse to administrator. Most have at least a Masters degree in Health Services Administration, although some pursue degrees in public or business administration. Hospital administrators with financial backgrounds may go on to become Chief Financial Officers or Chief Executive Officers.

Education includes a year of interning in a health care center, and most administrators begin as administrative assistants for large hospitals, before pursuing higher positions in smaller institutions. Therefore, it’s unlikely that Akalitus would be working alongside her former peers, as she probably would have needed to move elsewhere to find a position.

In another episode, Akalitus stays late to oversee Jackie making the time schedules, having noted that she always seems to be working with friends. One of the responsibilities of the hospital administrator is the maintenance of the budget. The hospital administrator would be responsible for the schedule of the nurses being overseen.

Finally, Gloria Akalitus appears to work alone. This is definitely true. There is often only one hospital administrator in a small hospital, and larger hospitals need a single administrator for each department. All Saints, the fictional hospital at which Nurse Jackie works at, seems like a large hospital. As a result, it would be common for a health care administrator to oversee the entire department alone.

These surgical tools have to be in very good condition and sterile as they are crucial in preserving doctor and patient safety by preventing infection.

When you add together the value of all of the equipment needed in a hospital, it comes to quite a large sum. This is the reason why many hospitals decide to lease their equipment instead of buying it. The first thing this saves, is the money that would be needed to make the large capital purchases. This money can be used to help their patient care sector improve. Lenders who are involved with hospitals often make their leasing agreements as flexible as possible. This helps the firms out greatly. As within a lot of sectors, the state-of-the-art technology in hospital equipment changes rapidly. Par Level Vs Kanban Methods – Which One For Hospital Material Management?

We have uncovered an opportunity that could mean millions of dollars in savings to individual hospitals, and billions of dollars to the healthcare system nationally in the US and abroad. It has to do with how most hospitals manage supplies, medications and other materials.

Many, maybe most, hospitals manage their inventory of supplies and medications using what is called a “par-level” method. It works like this: a stocking quantity is established for each item, the par level, based on average usage and a target number of days supply. We might, for example, set a goal of maintaining a two-day quantity of material for each supply item. As the material is actually used, we would bring the quantities “up to par” daily, by conducting a physical inventory and restocking the quantity that was consumed. The goal, sensibly, is to not run out of supplies while maintaining a tight control of storage space and inventory quantities. So far so good.

It is interesting to note that this par method of inventory control is not used in a world-class manufacturing environment, although a manufacturer certainly has the same needs and goals for inventory control as a hospital. The suggestion that we do a daily physical inventory for a large number of inventory items would be greeted with astonishment and ridicule. Many world-class manufacturing companies do not even conduct an annual inventory, by sustaining a high level of inventory accuracy through tight controls and cycle counting.

The method of choice in manufacturing for commonly used items is called Kanban. In a Kanban system, as with the par level method, we set a target quantity that we want to maintain. The principal difference is that instead of attempting to bring quantities “up to par” daily, in a Kanban system we set a fixed quantity that we will use to trigger the replenishment of inventory. In a “two-bin” kanban system, for example, we set up two quantities or bins of the same supply, and only refill a bin when it is empty. While the bin is being refilled, we have a second bin to cover usage during the replenishment cycle.

The Kanban method has seven main advantages over a Par-level system:

1. No daily counting is needed. We wait for a bin to be emptied and always replenish the same quantity. Not having to count can save hundreds or thousands of hours per year in most hospitals.

2. It reduces the number of resupply trips. Since we do not refill a Kanban bin daily, but instead wait for it to be empty, the number of replenishment trips can be reduced significantly. The number of replenishment cycles can be cut by 50% or more.

3. Replenishment quantities are fixed. The refilling process is greatly simplified by eliminating the need for counting required by the par system. If we know ahead of time what the refill quantity will be, the item can be stocked in that quantity.

4. It is easier to manage and improve. By tracking the time between replenishment, the stocking quantities can more easily be refined and adjusted over time. This continuous improvement is more difficult to accomplish if all quantities are refilled daily, in varying quantities.

5. Kanban reduces inventory. Experience proves that, with the same target coverage of supplies, a Kanban system will run with up to 50% less inventory than a par system.

6. It is easier to maintain replenishment discipline. Since they do not have to count all inventory locations, or eye-ball the empty bins, supplies handlers find it easier to identify and refill the empty bins, thereby substantially reducing the opportunities for shortages.

7. Kanban promotes good inventory management practices, while the par level does not. In fact, counting everything is essentially impossible and very labor intensive, and most par-level users simply “eye-ball” the bins without counting. Organization and housekeeping, “5S” in lean terms, is much easier to maintain.

For all of these reasons, Kanban is the method of choice for hospital material management, for much of the material that is procured and managed. The gains in productivity, reduced shortages and reduced inventory represent a multi-billion dollar opportunity for the industry.
